Medicare pays for skilled services, such as nursing and therapy in the home, when there is a skilled need and the patient is homebound. In order to qualify for home health service benefits, the patient must be under the care of a doctor who certifies the individual needs intermittent skilled nursing care, physical therapy, speech-language pathology services, or continued care assistance. A doctor also must certify that the patient is homebound.
